Package Plan Policies

1. Discounted Self-Pay Packages
Care packages are discounted self-pay plans. The package price reflects a bundled discount and is not based on the individual fee for each service or visit. Package services are not submitted to or billed to insurance, Medicare, or other third-party payers. By purchasing a package, you voluntarily elect the discounted self-pay option rather than billing the included services through insurance.

2. Payment & Expiration
Packages must be paid in full at the time of purchase. Packages expire 24 months from the date of purchase. In appropriate circumstances, a package may be paused when medically necessary.

3. Non-Transferable
Package visits are non-transferable and may only be used by the patient who purchased the package.

4. Refunds
Packages are refundable. If a refund is requested after visits have been used, the used visits will be recalculated at the applicable regular, non-package rate. The remaining unused balance will then be refunded.

5. Re-Examinations
Re-examinations are required every 6–8 visits or after 90–180 days without care, as clinically appropriate.

6. What's Included
Unless otherwise noted, each full-service follow-up visit includes chiropractic adjustment, electrical stimulation, moist heat, and bodywork/massage. Services may be modified or omitted when clinically contraindicated.

7. Adjustment-Only Visits
Adjustment-only visits are available for patients who prefer a streamlined visit without the additional in-office therapies included with full-service care.

8. No Guarantee of Results
Prepayment for a package does not guarantee any specific result or outcome. Services are provided on a per-visit basis and are not based on a guaranteed outcome.

9. Patient Choice & Insurance
By purchasing a discounted self-pay package, the patient acknowledges that they have been informed of their option to use applicable insurance benefits and voluntarily elect to receive care under the discounted self-pay package instead.
